Insurance: Jungle Roots Children’s Dentistry is happy to file dental insurance claims as a courtesy to our patients.  Understanding dental insurance coverage and benefits can be complicated and confusing.  We do our best to gather this information from the insurance company; however, there is never a guarantee that the information provided by the insurance company is complete and accurate.  We  advise our insured patients to contact their insurance company prior to a dental appointment and request a breakdown of dental benefits which  provides a  better understanding of insurance coverage.

When a dentist is contracted with an insurance company they are considered “in-network” providers.  Conversely, when a dentist is not contracted with an insurance company they are considered an “out-of-network” provider.  Patients are allowed to select their own dentist when an insurance plan is denoted as a “PPO” plan.  We advise insured patients to contact their insurance company in this case to determine the difference between “in-network” and “out-of-network” benefits and coverage.

As a contracted dental provider Dr. John E. Culp III, DMD is an “in network” provider for the following insurance companies:

DELTA DENTAL PREMIER PROGRAMS

GUARDIAN DENTAL GUARD PREFERRED SELECT NETWORK

CIGNA DENTAL PPO NETWORK

HUMANA DENTAL

BLUE CROSS/BLUE SHIELD OF ARIZONA

We accept and can file claims on behalf of patients for all other major PPO dental insurance plans but in those cases our office is considered an “out of network” provider.

Financial: Payment in full is expected at the time of the dental visit.  Patients with dental insurance must provide our office with complete insurance information in order for us to verify eligibility and file a claim for dental services rendered.  We are happy to file a dental insurance claim as a courtesy to our patients.

Routine visits are typically a covered benefit for most insurance plans; however, not all plans cover at 100%.  A statement is sent to the guarantor listed on the family account if a balance remains after the insurance company pays.  Payment is expected upon receipt of the statement.

When treatment is diagnosed by Dr. Culp a treatment plan is developed and presented to the parent attending the visit with the patient.  Insurance information provided by the subscriber/member is used to develop a treatment plan reflecting estimated patient portions and insurance portions.  Patient portions are only an estimate.  Actual out-of-pocket expenses may vary due to lower/higher insurance payments or changes in services rendered during the course of treatment.   The estimated patient portion is due and collected on the date the services are rendered.  These fees may include deductibles, co-payments or certain procedures not covered by the insurance policy.
